The ADESLB is designed to help teachers complete the 90 day UNPAID waiting period prior to utilizing Long Term Disability. These are calendar days that ignore weekends and holidays. It is contiguous time that is initiated by a physician/psychiatrist/psychologist’s date of disability. By contrast, Accumulated Sick days are actual teacher workdays. The use of Accumulated Sick Days is explained in the chart below.
To apply for us of the ADESLB, two forms must be completed. Both are can be printed out below. Complete your portion (Teacher Form). Have your physician/psychiatrist/psychologist complete the other form (Physician Form). Both can be faxed to dave Sweeney at (810) 538-2350. Once all paperwork is received, a district email is sent to the Administration, teacher and Sick Bank chair to officially initiate the use of ADESLB.
